Medical

Pharmacy Benefits
Capital RxPrescription drug benefits are provided through Capital Rx and are included automatically when you enroll in a Tobie medical plan. Prescriptions filled at Tobie or Walgreens retail pharmacies have more favorable copays. Specialty drugs (Tiers 4-6) are filled by the Tobie Specialty Pharmacy.

- HDHP Rx OOP max combined with medical: $6,000 individual / $12,000 family
- HDHP: Preventive drugs at no cost; all other drugs subject to deductible then 10% coinsurance (Tobie/Walgreens)
- Premier Plan generics: $5 (30-day) / $12 (90-day) at Tobie/Walgreens
- Standard/Premium Plan generics: $10 (30-day) / $25 (90-day) at Tobie/Walgreens

Generic drugs are mandatory unless a DAW waiver is prescribed by your doctor
The cost difference between brand and generic is not covered under copay or OOP limits
Non-Walgreens retail pharmacies have higher costs and a $150 brand drug deductible
Dental Benefits
CignaChoose between two levels of dental coverage: Base Plan and Enhanced Plan. Both are administered by Cigna and cover preventive care at 100%. The Enhanced Plan also covers orthodontia with a $2,000 lifetime maximum.

- Base Plan: lower cost, no orthodontia, annual max starts at $1,000 (increases $100/year with preventive use)

- Basic services: you pay 20% (bridges 50%)
- Major services: you pay 50%
- Endodontics, periodontics, oral surgery: you pay 20%
- Non-network annual maximum for Base Plan starts at $900 (Year 1)

Vision Benefits
VSPThe vision plan is administered by VSP and covers annual eye exams, lenses and frames or contact lenses in lieu of eyeglasses. Many in-network services are covered in full or require a copay, and a plan allowance is associated with many out-of-network services. Discounts are available on laser vision correction, additional glasses, sunglasses and lens enhancements.

FSA / HSA
WEXSave pre-tax dollars for healthcare and dependent care expenses. FSAs are administered by WEX. HSA accounts are available with the Blue HDHP and roll over year to year.
- Healthcare FSA: up to $3,300/year (2025 limit; 2026 TBD)
- Dependent Care FSA: up to $7,500/year
- HSA (HDHP only): $4,400 individual / $8,750 family (2026)
- Tobie contributes $750/individual or $1,500/family to HSA
- HSA funds roll over and are portable
FSA balances do NOT carry over. Unused money is forfeited after the claim deadline.
Life & AD&D
The HartfordTobie provides company-paid basic life insurance at 1.5x base pay. Supplemental life and AD&D coverage available for you and your dependents.
- Company-paid basic life: 1.5x base pay (up to $1,000,000)
- Supplemental life: up to 5x salary (max $1,000,000)
- Guaranteed issue up to $500,000
- AD&D: $25,000 to $500,000
- Spouse and child supplemental options available

Disability
The HartfordShort-term disability is voluntary (team member-paid). Long-term disability is company-paid. Both protect a portion of your income.
- STD: 60% of base pay, up to $2,500/week (team member-paid)
- LTD: 60% of base pay, up to $15,000/month (company-paid)
- STD default: 30-day elimination period
- LTD: 90-day waiting period
- STD paid after-tax so benefit is tax-free

Well-being Programs
All team members are encouraged to participate in well-being activities. Benefits-eligible team members can earn incentives including HRA contributions. The Healthy Lifestyles Program provides tools for managing diabetes and hypertension.

Understanding Your Benefits
Key terms explained in plain language to help you make informed benefit decisions.
Premium
The amount you pay each pay period for your health coverage. This cost is typically deducted pre-tax from your paycheck. Your premium depends on the plan you choose and whether you cover just yourself or include dependents.
Deductible
The amount you must pay out of pocket for covered services before your insurance begins to pay. For example, if your deductible is $1,500, you pay the first $1,500 of covered services. Preventive care is usually covered before you meet your deductible.
Coinsurance
The percentage of costs you share with your insurance plan after meeting your deductible. For example, if your plan has 80/20 coinsurance, the plan pays 80% and you pay 20% of covered services until you reach your out-of-pocket maximum.
Copay
A fixed dollar amount you pay at the time of a covered service, like a doctor visit or prescription. Copays vary by service type. For example, $25 for a primary care visit or $50 for a specialist. Copays count toward your out-of-pocket maximum.
Out-of-Pocket Maximum
The most you will pay for covered services in a plan year. Once you reach this limit, your plan pays 100% of covered services for the rest of the year. This includes deductibles, coinsurance, and copays (but not premiums).
HRA (Health Reimbursement Arrangement)
An employer-funded account that reimburses you for eligible medical expenses. HRA funds help offset your deductible costs. The HRA is owned by the employer and typically does not carry over or transfer if you leave.
HSA (Health Savings Account)
A tax-advantaged savings account available only with a High Deductible Health Plan (HDHP). You contribute pre-tax dollars, and the funds can be used for qualified medical expenses. HSA funds roll over year to year and are fully yours, even if you change jobs.
Networks & Tiers
Insurance plans organize providers into networks or tiers. Tier 1 (in-network) providers have the lowest costs for you. Tier 2 and Tier 3 providers are still covered but at higher costs. Going out-of-network means the highest out-of-pocket expenses. Always check your plan's provider directory.

Find Care Near You
Use the BCBS provider search to find in-network doctors, specialists, hospitals, and facilities. Staying in-network means lower out-of-pocket costs and better coverage. You can also search for dental providers through Delta Dental and vision providers through VSP.

Open Enrollment is held in November each year. Benefits elected during Open Enrollment become effective January 1 of the upcoming plan year. You have 31 days to enroll in Infor and submit required documentation if enrolling dependents in medical coverage.
Benefits-eligible team members include full-time employees classified as working 30 hours or more per week (minimum 0.75 FTE) and part-time employees working 24 to 29 hours per week (0.6 to 0.725 FTE). Part-time employees pay higher premium contributions. PRN team members are not eligible for well-being incentives. New hire flexible benefits begin on the date of hire.
If you do not actively enroll, you will be assigned a default package that includes only: basic life insurance (company-paid), long-term disability insurance (company-paid), and short-term disability insurance with a 30-day elimination period (voluntary, team member-paid). All other benefit elections, including medical coverage, will default to "waive." You must take action to elect a medical plan.
A qualifying life event (QLE) includes changes in marital status, change in the number of eligible dependents (birth, adoption), or other IRS-qualifying events. If you experience a QLE, you must make benefit changes in Infor within 31 days of the event date and upload verification documentation. Changes must be consistent with the qualifying life event. Changes are effective on the date of the event.
Enroll through Infor HR. On a work computer: from I-Connect, select Tools and Services > Team Member Services > Infor HR & Workforce Management. On a personal computer: go to Tobie.org/Team-Members and select the Infor link. If accessing outside the Tobie network, you will need Microsoft Authenticator. Call the IT Service Desk at 866-966-8268 for help with Microsoft Authenticator.
Once you enroll, you may not cancel or change your elections until the next Open Enrollment period unless you experience a qualifying life event (QLE). Examples include marriage, divorce, birth/adoption of a child, or loss of other coverage. You have 31 days from the event to submit changes in Infor with supporting documentation. Changes must be consistent with the qualifying life event.
